Who submits the application?

You should submit the application if you are a family member of an EU citizen and you do not have the nationality of a Member State of the European Union or of a Member State of the European Free Trade Association (EFTA) – a party to the agreement on The European Economic Area or the Swiss Confederation.

 

When should I submit the application?

You may submit the application in person, to the Voivodship Governor competent for the place of residence, no later than on the next day after the expiry of 3 months from the date of entry into the territory of the Republic of Poland. The requirement of personal submission of the application does not apply to the minor.

In the event of a permanent residence card: after the expiry of 5 years of continuous residence in the territory of the Republic of Poland with the EU citizen.

 

How can I submit the application?

on: Monday, Tuesday, Thursday, Friday. 9.00-16.00; Wednesday 8.00-18.00

After the arrival to the Office proceed to the ticket machine of the queue system and collect a ticket, and then wait for the call of your number and go to the indicated position.

 

What documents must I submit?

  1. the application for a residence card of a family member of an EU citizen (Application for issuing a residence card), (Application for issuing a permanent residence card), completed legibly in block letters in the Polish language (attachment of 2 extra copies of application definitely will speed up the procedure),/li>
  2. travel document (2 copies)
  3. 5 current coloured photographs, undamaged, with dimensions of 4, 5 x 3, 5 cm, taken within the last 6 months, on a unitary bright background, with good sharpness and showing clearly your eyes and face from the top of the head to the top of the shoulders; the face must cover at least 70-80% of the photography; the photography must represent a person looking straight with open eyes, shoulders not obscured by hair with a natural expression of face and closed mouth, as well as reproduce the natural colour of the skin.
    A Foreigner with congenital or acquired defects of the organs of sight may include a photograph showing them with dark glasses.
    A Foreigner wearing a headgear in accordance with the principles of their religion can include photography in headgear. Headgear must not cover or distort the contours of one’s face.
  4. Certificate of registration of residence of a citizen of the European Union, issued to the citizen of the European Union, with whom the member of a family is residing in the territory of the Republic of Poland.

In the event of a permanent resident, in addition:

  • Documents confirming the 5 years of continuous residence in the territory of the Republic of Poland with the EU citizen./li>

In addition, you must provide:

  1. When the application concerns a spouse - proof of marriage (i.e. marriage certificate),
  2. When the application concerns a descendant - a document confirming the existence of family ties and a document confirming age or certifying remaining dependent on a European Union citizen or on his or her spouse (e.g. birth certificate),
  3. When the application concerns an ascendant - a document confirming the existence of family ties and a document confirming age or certifying remaining dependent on a European Union citizen or on his or her spouse.

 

What are the fees?

In accordance with the Act of April 1, 2011, on the amendment of the law on entry into the territory of the Republic of Poland, stay at and departure from this territory of citizens of the Member States of the European Union and members of their families (Journal of laws of 2011 no. 92, item. 532) in those cases no fees will be charged.

The receipt of the document should be made by the applicant or a representative holding a special power of attorney to collect it.

If you changed your place of residence, personal information, lost or damaged the document or its validity expired - submit PERSONALLY an application for issuance/exchange of residence card of a family member of an EU citizen or issuance/exchange of a permanent residence card of a family member of an EU citizen.
